An additional thought (since additional thinking is never a bad thing)...
Americans have access to the best and most expensive medical care in the world, but the American population is far from being the most healthy or health conscious because of it. Having to pay for a doctor's visit, which is a staple of the American system, has not prevented the United States from experiencing an epidemic in obesity, diabetes and heart disease unlike any other country on earth.
